The Critical Conversation Before Starting

If you're considering TRT and want children now or in the future, this is the most important page you'll read. Exogenous testosterone suppresses your body's natural hormone production — including the signals that drive sperm production.

How TRT Affects Sperm Production

Your brain regulates testosterone production through the HPG axis: the hypothalamus signals the pituitary gland (via GnRH), which releases LH and FSH, which tell the testes to produce testosterone and sperm. When you introduce external testosterone, your brain detects adequate hormone levels and reduces these signals. The result: your testes produce less testosterone AND less sperm — sometimes to zero (azoospermia).

Is It Reversible?

For most men, yes — but with caveats. After stopping TRT, natural production typically recovers within 6-12 months. However, recovery time varies significantly. Some men take longer, and in rare cases, full recovery doesn't occur. Age, duration of TRT use, and baseline fertility all affect recovery.

Fertility-Preserving Alternatives

Clomiphene citrate (Clomid). A selective estrogen receptor modulator that stimulates your body's own testosterone production by blocking estrogen feedback at the pituitary. Raises testosterone while maintaining — or even improving — sperm production. A good option for men with mild-to-moderate low T who prioritize fertility.

Enclomiphene. The active isomer of clomiphene, with fewer side effects. Increasingly prescribed as a TRT alternative for fertility preservation.

HCG co-therapy. Human chorionic gonadotropin mimics LH, maintaining testicular stimulation even while on TRT. Some providers add HCG to TRT protocols specifically to preserve testicular function and sperm production.

Sperm banking. For men who want TRT now and may want children later, banking sperm before starting treatment provides an insurance policy regardless of what happens to natural production.

The Bottom Line

Never start TRT without discussing fertility with your provider. If children are in your future, there are effective alternatives and combination strategies. This conversation should happen BEFORE your first dose, not after.
