How to Inject Testosterone: Complete Step-by-Step Guide
Self-injecting testosterone is easier than you think. Most patients say it takes under 60 seconds once they get the hang of it. Here's exactly how to do it safely.
Before You Start
Your Heyday's TRT program care team will walk you through your first injection during onboarding. This guide is a reference for ongoing self-administration. Always follow your provider's specific instructions if they differ from this general guide.
What You'll Need
Testosterone vial (testosterone cypionate 200mg/mL), draw needle (18-20 gauge), injection needle (25-27 gauge, 1" or 5/8"), alcohol swabs, gauze pad, sharps container. All included with your Heyday kit are included with your Heyday prescription.
Step-by-Step Injection Guide
Wash Your Hands
Thoroughly with soap and water for at least 20 seconds. Clean the injection site with an alcohol swab and let it air dry.
Prepare the Medication
Wipe the vial top with an alcohol swab. Draw air into the syringe equal to your dose amount, inject the air into the vial, then invert and draw your prescribed dose. Remove air bubbles by tapping the syringe gently and pushing the plunger until a small drop appears at the needle tip.
Switch Needles
Replace the draw needle with the smaller injection needle. This makes the injection significantly more comfortable — the draw needle is larger to pull the thick oil-based testosterone through the vial stopper.
Choose Your Injection Site
Intramuscular (IM): outer thigh (vastus lateralis) or deltoid (shoulder). Subcutaneous (SubQ): abdominal fat pad or outer thigh fat. Rotate sites each week — don't inject in the same spot consecutively.
Inject
For IM: insert the needle at a 90-degree angle in one smooth motion. For SubQ: pinch the skin and insert at a 45-degree angle. Inject slowly and steadily — take about 10 seconds to push the full dose. Rushing can cause post-injection soreness.
Withdraw and Apply Pressure
Remove the needle in one smooth motion at the same angle you inserted it. Apply a gauze pad with light pressure for 30 seconds. A small amount of blood is normal. Apply a bandage if needed.
Dispose Safely
Place used needles immediately into your sharps container (included with your kit). Never recap used needles. When the container is three-quarters full, seal it and dispose according to your local regulations or return it with your next Heyday shipment.
Intramuscular vs. Subcutaneous: Which Is Better?
Both methods are effective and clinically validated. IM injections have been the standard for decades and deliver testosterone directly into muscle tissue. SubQ injections are newer in TRT practice but gaining popularity because they use smaller needles, are less intimidating, and some studies show more stable testosterone levels with fewer peaks and troughs.
Your Heyday provider will recommend the method that's best for your protocol. Many patients start with IM and switch to SubQ once comfortable, or vice versa.
Common Concerns
"Will it hurt?" — Most patients describe a brief pinch, similar to a flu shot. The 25-27 gauge injection needle is very thin. After a few weeks, most men say they barely notice it.
"What if I see blood?" — A small amount of blood at the injection site is completely normal. You may have nicked a small capillary. Apply pressure and it will stop quickly.
"Post-injection pain?" — Mild soreness at the injection site for 1-2 days is common, especially in the first few weeks. This typically resolves as your body adjusts. Injecting slowly and warming the vial in your hands for a few minutes before injection can help.
"What if I miss a dose?" — Inject as soon as you remember. If it's close to your next scheduled dose, skip the missed one and resume your regular schedule. Don't double up. Contact your provider if you miss more than one dose.
