Could it be low testosterone?

Quick answer: The common symptoms men associate with low testosterone — fatigue that sleep does not fix, lower sex drive, loss of strength, weight gain around the middle, low mood and poor concentration — are also caused by sleep apnea, thyroid disease, depression, alcohol, medications and iron deficiency. Symptoms alone cannot tell you which. Only a blood test, read by a clinician alongside your history, can.

1. You wake up after a full night and still feel unrested.
2. Energy falls off a cliff in the early afternoon most days.
3. Interest in sex is noticeably lower than it used to be.
4. The same training gives you less than it did two years ago.
5. Weight is settling around your middle despite no big change in habits.
6. You are flatter, shorter-tempered or less motivated than usual.
7. Concentration slips and you lose the thread more than you used to.
8. You snore heavily, wake often, or have been told you stop breathing.

What else causes these symptoms

Before you conclude it is hormones, these are worth ruling out — a good clinician checks them in the same panel or the same conversation:

  • Sleep apnea. It produces nearly the whole symptom list on its own, and it also lowers testosterone. See tired after eight hours of sleep.
  • Thyroid disease. Fatigue, weight change, low mood, cold intolerance. A TSH is cheap and settles it.
  • Depression and chronic stress. Overlapping symptoms, different treatment. See testosterone and mental health.

  • Iron, B12 and vitamin D. Ordinary deficiencies, ordinary fatigue.
  • Medications. Opioids, some antidepressants, finasteride and steroids all affect hormones.
